Edmund McMillen thinks about it all. The game is very connected with his life and knowledge about the Bible which he has A LOT. I've read a few interviews with him and such and he doesn't seem like a very intelligent and interesting person.

Did he just compare Catholicism with Dungeons & Dragons? I am not joking right now, I feel a bit offended for D&D. It's a legitimate game that highly influenced modern fantasy! But yeah, I see his point.

It does seem legit. It was from a book that documented folk beliefs of the first American settlers or something. I forget the name, but I think the author just wandered around America asking lumberjacks and farmers to tell him what stories they knew, and they probably made stuff up on the spot.
