At the top of the page (right above the featured deviation and the journal) there are the bottons that say "General" , "Gallery", "Scraps", "Journal", "Activity", "Send Note", and "DeviantWatch"
Click the one that says note.
Then, where it says "Recipient(s):" type the username of who you want to send your note to. (It normally already says it.)
Then.. er. The rest is pretty easy to figure out.
