I'm not delving into that FNAF 2 prequel theory, I have been in WAY to many arguments over that.
My personal theory is Toy Freddy, alright?

Well, many assume that it was either Foxy or Mangle due to their sharper teeth, to bite it out. But really, they never sad it WAS bitten out! All we know is
A. There was a biting
B. It was in 1987
C. Someone, as a result, lost their frontal lobe.
Now this would make most people believe it was torn right out, but with what we know, another possibility could be that the animatronic bitten down so hard, it cracked through the skull, and the lobe had to have been surgically removed. Think of the animatronic in question as a nut cracker and the shell of the nut as the skull. It had to have been crushed between two flat surfaces, like Toy Freddy's teeth, Compare them some others who have been accused: Notice anything? All of them, except for Toy Freddy, have noticeable gaps in there teeth, while Toy Freddy, while having similar gaps, are much more closed in and a little more flattened. Also take in mind Toy Freddy's mouth can open up noticeably wide. Toy Bonnie's mouth is shown not to open very far, Toy Chica most likely has her beak on all day, and Mangle never closes her mouth, hinting her jaw could be broken. Put this with the FNAF prequel theory and it holds up rather strong!
But I guess all these theories won't mean anything when the fourth game is released!

That actually makes sense! Toy Freddy could have used his suit teeth to break the skull then in the process use his endo's sharp teeth to tear that lobe out! FNAF4 is definitely about the bite because of the "Was it me?" text.

Ah ha! But theres one whole in this entire thing! It had to be an animatronic fast enough to do it without anyone seeing them cause in-game they said the animatronic was unknown. Yes, im heading back to Mrs. Spiderman:

Well, on the topic of speed, I have to give that to Toy Freddy can practically disappear, as seen as when he fakes you out in the second game. He's in your office and his eyes are black, the room goes dark, he apparently goes away, until you see him back only a few seconds later when you put the monitor up. But speed aside, look at one of the last frames before her jumpscare ends

Notice anything, here front and even back teeth seemed to be rounded. Again, this is just a theory. You can believe whatever you like.
But hey, you can at least admit Mangle can be adorable sometimes, as clearly evidenced!

True, true... Wait. All of the animatronics just disappear, right? I have something for my theory thanks to that picture! As you know. First-person view is always the picture of only the eyes. Where Mangle's endo mouth is adjusted to is higher up and not in the middle. This could mean that Mangle is trying to bite your head. Also, mangle is the only one that legitimatly trys to bite you. Although Old Freddy does too. Well it's a theory...
Any secrets on the last frames of Freddy's jumpscare cause Toy Freddy looks like he's staring directly at you. (If only I knew what he was doing to you? Strangle? Give heart attack? Punch?)
