My visualization of Arkham Asylum's Paperwork. This is an Arkham Form 603. It's a Neurological Examination form that is part of a patient's medical record. I drew my inspiration from the Standard Form 530 and I developed the Arkham Logo based off of Rocksteady's Arkham Logo from Batman Arkham Asylum.
