It's mostly traditional, but I added some ambient lighting digitally.Here's Link & Tetra, sleeping with their two kids, Finn & Shelli.

I use one of the soft photoshop brushes, and set the minimum diameter all the way down. Well, that, and I use the polygon mask/selection tool, sometimes I feather it...
I'm usually terrible when it comes to explanations, I hope I answered something here...
