You save it/copy it and open it in an art program like Photoshop or Gimp (or paint) and draw the characters in each box doing their responses to the questions(just look at other memes for example). Then save it as a .png or .jpg and put it up.
Or, if you don't have an art program/aren't good at using it you can print it off and draw it in by hand(or do the drawings by hand, scan them in and put them to the boxes)
