I guess it's about pain, since there's loss of blood and the thing in the middle is some kind of weird disease (physical pain), plus the red dots are apparently leaving, getting away from the disease so that might be the loss of important elements in your life like friends or family, which cause pain too, at another level (but that's less obvious).
