This was made with a gas mask + a modified flash unit that fit inside of the mask itself. Smoke was blown from the models mouth.

This is such a cool shot. It's got so much that works for it. The softness of the model exposed as she opens her jacket, the reflected mask, the pose and the smoke work perfectly. Overall, this is great.
