When theΒ Tails DollΒ Goes to Maksa's House to Kill Maksa, he awoke the Black Ghost he murdered. Terrified by the Black Ghost, Tails Doll ran into the Basement where Maksa wasΒ Basement. He wore the Body to hide from the Black Ghost, and the trick appeared to have worked at first, causing him to stand up suddenly and laugh at the Black Ghost while inside Maksa.
However, the mechanisms within Maksa malfunctioned, Smiley The Killer Right Behide Tails Doll, Tails Doll Gets Stabbed by Smiley The Killer, thus killing him. James's tapes also foreshadow this, as he explains that the person wearing the costumes must not breathe on the spring locks or make sudden movements, and Tails Doll broke these rules; he made a sudden movement by jumping into the Body, and possibly breathed on the spring locks when he laughed, resulting in both the locks becoming loosened and his brutal death.

wait thats what happened to the Purple Guy when he became SpringTrap with the wholeΒ wearing the costumes must not breathe on the spring locks or make sudden movements, although a backstory of how TD became THIS beat up and shit would have been awesome to see
