Basically the war never happened and the Humans and Monsters continued to live in a peaceful way, until a virus was discovered, this virus makes the monsters transform into nightmarish and disturbing creatures which also makes them highly aggressive.
So the Humans and Monsters that didn't have the virus had to prepare for this and they managed to push all the infected Monsters to the underground and seal them.
Another thing to say is that when the infected monsters are in their normal state, they are even more friendly than before, but they become very aggressive when they transform, this can happen at any time or when they are very stressed, something simple.

Also when they are in their infected form, some colored worms with many mouths or empty eyes can grow in any part of their body, Sans for example has some of them growing in the back of his skull.
