First of all there is a wonderful elegance to this design. The forms of the hands in particular are very nicely shaped and positioned. The hair, while wild, has taken on that kind of amorphous floating quality as if it was underwater, which is its own interesting addition to this piece!
Another part I like is the criss-crossing shading smudges below the chin line. The face itself is so smoothly colored, and then that loss below the jaw is intriguing. On any other piece, I might comprehend it as either an unfinished portion, quick-finishing laziness, or something else entirely. Yet here, with this theme and your description, it clearly speaks to a loss of the smooth control. As if there is such a line where things are held in check and the road is smooth for moving forward... and then suddenly, all at once perhaps, the road is riddled with potholes and bumps and direction is at a loss (such as the criss-crossing). I'm probably reading too much into that, but that's what it made me think.
I really like this piece, regardless... I see some of myself in it.

It is your own hand that is holding you down, and your own hand that is loading your with burdens. I don't say, "free yourself," because that is not easy. But keeping in mind who is limiting yourself will eventually shift the balance: you will either let yourself go - or you will accept that this is what you want. Either way you will feel more content.
