Very nice! the only thing that doesn't fit is that the other slice of bread is actually inside the toaster. Other than that is just great :]

But I have thought about doing this shot on location possibly in a kitchen, and having the other one jumping out as well, but still down further from the other piece of toast that is in focus

it was shot with a large format camera. the camera was aimed at the ground. The toast was suspended on a boom rod, and held up with bent paper clips that held it in place. after that, add in a couple hours and ta daa!
