After Tako relinquished the belt Cepha was first in line to take it, and take it she did. She's held on to the belt for two years strait, but over time untreatable crustacean parasites have worn her down. She still defends her title through her sickness. holding onto the hope that one day she would get to fight Tako again before she dies.
