Ah, that's one of the endings I wish to happen in the manga: going blind. But here is the others Sasuke-ends-up-helping-Naruto-against-Madara but something happens scenarios:
1- Sasuke going vegetal because of Madara's Magekyou sharingan
2- Sasuke being paralyzed from waist down because Madara stabbed him right in his spine with his sword
3- Sasuke becoming a traveling monk after Naruto's death.
Well, my wish is he's going blind since to my view the sharingan only bring suffering and disgrace to Sasuke. sasuke 's better without the sharigan.

I agree the "blind" Uchiha is the worse fate they could have,

I know what you mean. I wish Sasuke going blind in the end of the manga. But knowing Kishie, nothing will happen in there, even a permanent damage in one of the characters. I mean, after everything Sasuke did in the manga and nothing happening to him in the end, the story will not feel real enought to me.
