We get the neutral ending! The prophecy is right: Twilight perishes, but she gets to say goodbye. I feared she would die given the dark turn of the last few pages. I was hopeful for the good ending (Twilight lives to tell the tale), but I was expecting this one.
It still hurts though. It hurts knowing that this is Twilight's reward for all her suffering and all her hard work: a chaotic, bloody reign by a faker, and death with her friends voices as the only consolation in her last seconds. She can't even have the pleasure of seeing their faces one last time. 'Tis a bitter end for such a hardworking pony who did her best to make the world a better place.
The only way this could turn out okay for her is if there's an afterlife in this world, and her friends and mentors are waiting for her in eternal peace and joy. Who knows? Maybe that's what the prophecy meant: Twilight wouldn't outlive her friends in Heaven. Instead, having shed the mortal coil, they'd be together forever, completely content and joyful and living a greater and fuller life than before. After all, Heaven (or some kind of similar afterlife) is at least acknowledged in S4's "Filli Vanilli" when Applejack said Fluttershy's singing was like a "slice of Heaven." I know it's a figure of speech, but figures of speech don't just appear out of the ether. They all have their origins. So who knows? Maybe the neutral ending is actually a good ending, but that would only be the case if there's an afterlife of some kind, an eternal reward as it were.
By the way, I'm not criticizing the story or author. Good stories should hurt when the need arises. That means we're invested in the characters and have to think through the themes being addressed. However, I will ask why Twilight told the doctor that her friends would misunderstand him when he said she was getting worse' There was nothing to misunderstand; she was dying. Unless this is like a Force thing or a Christian afterlife and she becomes a greater/purer being when she "dies." In that case, see the above paragraph.

I think she meant that her condition is hopeless, saying that the situation will get worse if her condition persists implies that there is a chance she could pull through and that she will only die IF it gets worse or continues and that otherwise she could live. No, Twilight knows her time has come and her magic, including her life force is utterly spent and the last bit of it is fading away as the magical damage breaks her body down. There is nothing that could possibly save her and the only reason she is still there is because she wants to say goodbye. At best her spirit will become one with the magic of friendship itself and live on within the hearts of her friends.
