Here it is: Skarlet Hydra’s second form.
A pity we never get to see D in his true form, so I have to customize it. Hints appear that since he’s an initiate, it could have been a small dragon. But I prefer making them big. It could have been four legged, who knows?
For basis of my idea of what his dragon form could look like, it could look like this but smaller and without Dargon bones. Have to come up with another headcanon if Dargon bones infects a dragon, that dragon will mutate and grow bigger to near the Dargon King’s size.

And before I forgot: About the new Hydra attacks, they are just D's limit breaks corresponding to each one (Ex. Hydra's Breath used to be Dragon's Breath) but 3x stronger. I was going to make them hit three times, but that would had made it too OP especially Hydra's Breath.
