That's kind of the point: Operating theatres were built so that students could observe procedures. If you want to be pedantic, if there is no gallery for observers it isn't an operating theatre.

I agree with the origin of the word choice. I'm simply pointing out that this particular image is emphasizing the theater aspect of the room, rather than the operating aspect.

So did the ones in the real world, for two reasons:
If you were well off, you got to have procedures done at home where you weren't made a spectacle of.
There was very little in the way of what we would refer to as "operating" back then. These things are from the days that surgery involved, (hopefully), dosing the patient up on narcotics and either straps or strong assistants to hold them in place. Anaesthesia only came in late in the use of those theatres and that was nothing more than sedatives rather than the mix of paralytics, pain-blockers and memory-blockers that are used today. The most you would see is an amputation that would be done very fast, (as in 'remove and suture a leg in under 60 seconds'), and almost no internal work outside of autopsies.
