Name: (First name, Middle Name of Your Form fighter Oc's Ancestor's Last Name, Your Form Fighter Oc's Birth Last Name)

Age: (Your Form Fighter Oc has to be 13 to Older)

Gender:

Ability (if Your Form Fighter Oc's Ancestor has Ability to pass on to their descendant)

Species: (They can be Humans, Kemonomimis, Normal Animals and Juujins)

Sexuality:

Personality Traits: (only 3 Positive, 3 Neutral and 3 Negative Personality traits for your form fighter ocs)

Phobias: (in the form fighter world, Form Fighters have a fear of something that leads to their traumas)

Is Your Form Fighter Pure born or Non-born: (if your form fighter is Pure form, they have to have an forcalus gem middle of their chest only where the closely above of the bone collar)

Forcalus Gem Accessory (if Your Form Fighter is Non-Born): (Forcalus Gem are only place in Necklaces, Bracelets, Hairclips, Earrings and rings but for animals, it's just pet tag on animal's collars)

Formling(s): (Your Form Fighter can only have 6 formlings slots, and always starts with one Formling randomize of which type of Formling that can shapeshifted your form fighter's appearance into)

Inventory: (this is where you puts your weapons,healing items and other items for your form fighter or your little formlings)

Backstory: (This is your Form Fighter Oc's Backstory of how they become an Form fighter and their traumatic or normal past soo keep it PG-13 please)
