Rinko doing some maintenance on the real world body of Alice's body. Rinko might have managed to have herself made pregnant with Kayaba's children sometime after his physical death in SAO.The story behind Alice being pregnant here would be that Asuna had some issues and her babies were born early. Alice volunteered to carry the babies the rest of the way for her and Kazuto within her robotic body.
For Alice I based her robotic frame on Niemom and Ichiemom, from SAO itself. Along with the Terminators from that series. Since there wasn't any clear images of what Alice's robot frame looked on the inside.
