According to Japanese Urban Legend, the Aka Manto (meaning: Red Cloak) is a malevolent spirit wearing a mask and the namesake cloak. This supernatural being haunts the restrooms of public bathrooms, especially schools, attempting to victimize the most vulnerable. The Aka Manto will approach a potential victim using the restroom and ask them if they want the red paper or the blue paper. Either choice is a deathwish. Choosing red will have the spirit stab the victim to death and their clothes will be soaked in blood. Choosing blue will have the victim be strangled to death until their skin turns blue due to lack of oxygen. Choosing any color other than red or blue will have the victim sent straight to the depths of hell. The only way to save oneself from this spirit is to ignore the questions (or even answering "nothing") and then run out of the bathroom. To know more, read this: Aka Manto - Wikipedia

The Japanese text on the screen is written by hand. They mean: "Red Paper Blue Paper". In the game, this would indicate that the Aka Manto is nearby or otherwise add the intensity of the spirit chasing the player.

TBH, I never played or owned the game because I'm too scared to play it and I watched many YouTubers play it.

The Aka Manto depicted here is just made from a Splatoon Inkling.

Because they picked up the red paper, the Aka Manto attempts to stab them with the knife (If it were blue, he would have tried to strangle them with his bare hands).
