Technically speaking, He was stabbed between the ribs into His heart to make sure He was already dead (which He was) to fulfill the prophecy that none of His bones would be broken (the Romans broke the legs of the others to hasten their dying).  And blood and water flowed out, medically we have discovered that blood and water (lymph) separate after death--also this would be a sign the sac around the heart had been ruptured, and this was further proof that He had already expired, but also providing further symbolism of His purifying blood (mercy) and living water (grace).
