This took me a while to do, but I finally got around it! Here's a top 30 of some of the scariest PSAs in my opinion. I didn't wanna add in the overrated ones such as Dark & Lonely Water and a couple others.
1: Haven 911 Call

This is one of the many PSAs that would make you wanna shout "JESUS CHRIST" and for a good reason at the end. In this horrifying ad, a child calls 911 about his or her mother being abused by her ex-boyfriend. You don't see anything except the visuals.
2: Kill Your Speed

A girl keeps telling the audience that "Your going to kill me one day" because of speeding all while looking at the camera every time she's in a different direction. This keeps going on until at the end

I hate this one. It doesn't help that this was one of the first few PSAs that I've witnessed. Throughout, there's a girl smiling and looking directly at us while "Happy Birthday" is playing in the background. It's fine, until a woman says in a creepy ass voice "How old are you now". As this is going on, the woman's face becomes more pale, while the narrator tells us about the warnings of taking drugs. The add ends with her lifeless face still staring at the camera.

We're shown in a nursery full of babies. All fine a first, but suddenly out of nowhere, we hear a woman, followed by a slideshow, until it stops at a gravesite at the end.

Alot of my friends would mostly pick Evan on their list, but I think Point of View is probably the more scarier one along with the one coming up.

To be honest, I'd thought this was a Target commercial, up until I heard screaming in the background then I realized what was going on.
7: RSPCA-My Little Puppy

Similar to Back To School Essentials, I'd thought this was gonna be a Pet Smart commercial, up until I saw the ending. Mainly it's just a PSA disguising itself as a pet commercial. But unlike most pet commercials, it starts off with "My Little Puppy comes completey un house trained, enjoys being taken on lots of walks, needs feeding on a regular bases and requires endless love and Attention". In the words of Ron Burgundy, That's where things exultate quickly as we see the father putting the poor puppy in a bag. You can guess what happens next there....

9: We Prevent Crime

We have a woman with her son in her arms while singing "Hush Little Baby" as the police are trying to comfort her. The father joins the picture and the two of them began to cry. It's revealed that their child was killed in some sort of way. The PSA ends with their child's funeral.

11: I've Lost Everything

It starts off innocent enough with a girl name Leah describing how she'd lost certain things while growing up such as her blanket, her dog and one of her teeth. Then it takes a turn for the worst as the caption says that at age of seven, she'd lost her innocence.

Ah, yes. You've probably know this one from the Nostalgia Critic as this was the start of the classic PSA gag in his commercial reviews. This one has the intermentioned baby shower, revealing that the gender of the baby is a girl. It starts off cute, until the mother opens a box to see that she had gotten a ahem, certain kind of whistle.
13: WISB-Sous Chef

Yet, another PSA that's so well known nowadays that I don't even have to describe this one, but I'll do it anyway. In this classic, a young chef mentioned that she'll be a sous chef and that she's gonna get married probably by the same next year. As she starts talking though, she picks up a pot with boiling hot water & trips over grease that's on the floor. Yeah, you can guess what happens next.

This one sounds a tad ridiculous when you first hear about it. But when you actually watch it, the PSA's actually quite creepy with the woman screaming loudly and the baby almost falling over the pram.
15: Electric Blanket-George & Betty

Starts off cute with a elderly couple having a romantic date, up until George pulls out an electric blanket.
16: Great Ormond Street-Hospital

This eerie PSA just shows what looks like a hospital and some children singing all with a creepy jingle.
17: RSPCA-How Much Is That Doggy

Another well known one. This PSA just has us looking at a dog while the titular song plays in the background. You have to wait until the end to see why it's so controversial.
18: S.H.A.R.P

We see a man getting dressed up for someone's funeral while talking about his daughter. It all seems good, up until the end where if someone hadn't been saved in time, his girl wouldn't have gotten AIDS. The music is something you don't wanna hear in the middle of the night.

Yet another song you don't wanna hear around at 12 AM, especially near the end where the child sings "Oh, God. We killed them all."
20: Carbon Monoxide

This one shows a woman at a night with some friends then coming home to turn on her heater before going to bed...only to reveal that she had died from carbon monoxide the next morning.
21: Save The Children-Goldfish

It shows a goldfish happily swimming in it's fishbowl. Until all of the sudden, a hand violently grabs it and puts it down without water.
22: ST John-Popcorn

The PSA begins with a history of popcorn, which is then followed by a family enjoying a movie night. All of the sudden, the girl accidently eats the popcorn fast and begins to choke. One of the audience exclaims that she's a nurse and runs in to save the daughter in time.

This one shows a man and a daughter seemingly happily coming home from school. The real horror? The man isn't her father.

The PSA Begins with two hands counting down to the New Year. Each hand is missing a finger until the end where everyone shouts "Happy New Year" and there isn't any fingers on the hand left.

The PSA Has relatives and friends at a child's party. As the mother comes outside with the cake, the child falls into the pool.
