I picture that the Sapper suffered from a form of white blood cell loss and as the Green Virus spread, the white blood cells mutated within the host, causing them to create a overload of white blood cells. Since the sac on the chest is located on the chest that either means his lungs also help produce the substance OR the sac is connected to the lungs, hence why he's damaged when he uses the sap
