Dead or asleep, I can't answer that because the doubt is a part of the composition
About the story behind it, I can tell you some things by the clues on the sceen. You may barely see some doodles on the wall (on the left), the little girl drew them, following those doodles, it looks like she found the craneman in a red factory. It looks like there is kind of order, look at the books, the curtain and the toysbox, perhaps she miss a past life, perhaps her family. Because of the doodles on him (the craneman) you may see they got kind of friendship, and she give him a cross as a gift. Then you may see impacts of bullets and a doodle of a tank on him, also a destroyed place, it looks like there was o there is still a war. Also he may got the impacts of bullet defending the little girl.
There are alittle more details to found, i just told you the most importants.
I can't tell you beyond this point.
