Nice comment and I agree with some of the things you said but I think you are over-analyzing this. I do not think that the illustrator had in mind the things you said. " He holds a phallic half of the turning bar with his bent left arm" No, he holds a wheel and they are not 36 ways to hold something. How else would he hold an object? between two fingers? "...with his bent left arm" Well yeah bent arms; how else? With the arms stretched?
What is a phallic half anyway? And if this was his intention why not a full phallic? Hey, it's just a bar. When I hold the shift on a manual transmission car I am not holding a phallus. You are reading too much in this. With that mentality holding a pencil is suspicious. Having a pencil or pen in your mouth as some people do when they are thinking, do not mean anything sexual. If a man does it, it is not a secret desire to lick a penis and therefore he is a homosexual. If a woman does it, it's a secret desire for... bla bla bla. By the way I'm a psychologist and the problem we often encounter is people who want to play psychiatrist/psychologist and psychoanalyze everything. A Volcano erupting in a story? Hey, that's clearly a reference to an erection!
Now as for the reason of the facial expression: The man is not "grimacing" (in pain for ex.) he is just smiling in a sadistic way.
I agree with you about the facial expression of the woman; surprise rather than horror and especially the expression of the woman in the background but you have to know how these drawings are made; Models are posing for the illustrator. They are usually not tied up but just keep their hands together (the illustrator add the ropes in the drawing). Sometimes they themselves twist a scarf around the wrists; they are still not tied up but it looks that way. They are models not actresses. And so the expression is not what you think it should be because of all this; They are not in character, tied up etc. they are sitting on a chair (the illustrator will change that to whatever he wants later, if they are laying on a sofa it will become a plank or some kind of torture device). An actress could give a better performance because she will be in costume, tied up on a device that is what the audience will see: If it's, say, a rail track it will be a rail track when shooting not 2 sticks put on the floor of the illustrator studio and telling the model "imagine it is a rail track."
If you look at other drawings the girls have often the same expression you describe.

The art of the cover is composed entirely for having a sexual/erotic subliminal effect, and is not to be taken literally.
The man operating the garrotte is grimacing and straining more than is the woman sitting in it. He holds a phallic half of the turning bar with his bent left arm. Male readers of this magazine are probably doing likewise (if they are left-handed).
Her expression is one of mild shock or surprise, as if all that is happening is that she is getting goosed back there, or she is having her bottom pinched, in public by an overly frisky male.
The male in the background looks proud of himself. Male reader, take note. This is your cue to do likewise for vicarious-thrill effect.
Finally, the woman chained to the wall stands with raised arms as if she is a cheerleader in all this. Her head is tilted as if she is appraising the scene thoughtfully, and fantasizing, mentally projecting herself as a participant. Which might well be true if she is next to take the seat at center stage.
Feminists, take note of the careful analysis above! You probably had been thinking all I would notice is the lady in red's cleavage and legs.
(well, I was. But do you think I am going to admit it? HA!)
