This are the forms mentioned on the SCP Article I did of omnitrix rias.her froms of SCP-745,SCP-312 and SCP-860-2.As for the nicknames.745 form: bright head
312 form: jelly cloud
860-2 form: forest cat
Btw,I know that not How 745 looks like.but I seen that people had draw that Species differently. So here is my Interpretation of it.also,one more thing.thanks to rias to be able to change that a form of another 860-2 ,she can communicate with it thru mind reading.that and said form can make portals to 860-1 (the forest).
