I don't see them as sisters, I have a headcannon of how alicorns are made and they simply became good friends during their rule to the point where they can call each other "sister" like people who've known each other from the time they were in grade school call each other sister or bro.

According to Faust, who doesn't work on the project anymore. I haven't watched any of the show sense season 3 so if that was something hinted at in the 5th season the ok, but I still have my own head cannon for how alicorns come to be and it's separate from the show.
