I just finished this one recently. As you can see, Sonic, Shadow, Rouge, and Tangle are in a hospital waiting room. Shadow seems... uncharacteristically utterly petrified. Wonder why...

I’ll give you the short answer: the guy’s afraid of shots. Think Loona from Helluva Boss season 2 episode 4. I have no shame. I’ve never drawn a truly terrified Sonic character (much less SHADOW OF ALL PEOPLE), but I think I conveyed the emotion well. I made Rouge’s boots too short and anatomy’s study is something we should all at least glimpse at, but I’m proud of it. The floor was actually what took the longest. I was having trouble deciding how to color it. So I just sat down, looked at waiting room flooring while listening to Sonic Prime until I got it.
