Yes, this is Madara with baby Nagato.

As you may have seen in Downfall: headcanon that Madara is Nagato's father and Mito is Naga's mother. He was the result of an affair they waited for years until they were certain there was no danger of caving a child (aka until Mito came to menopause...) but this didn't happen because of the Kyubi's chakra still remaining in her body, and the long awaited physical encounter did end up with pregnancy, that forced Mito out of Konoha fearing Tobirama, newly invested Hokage, could have killed the child if he had any Uchiha traits (which he did, inheriting the rinnegan).
Mito died at childbirth because of the old age, leaving Madara with a child he didn't want, a child he didn't know about until the woman came knocking at his door (madara was exiled remember... actually supposed dead...), a child that robbed him of the last thing he ever loved. And yet he couldn't hate his own blood... So he tried providing the child, specifically named Nagato(long lasting gate) as in memory of the parent's sacrifice and romance they waited to happen for so long, with a more capable family in a less known environment to Konoha, a place the Hokage won't search (like Ame).

So here we have Mads crying and recalling Mito, but holding protectively Nagato's newly born body, because he despises and loves this child. (This au personally rips my heart in half)
