You have a few flaws:
1. The egg space, you have to say WHERE it was hatched. Levels do not matter here.
2. The second place you put- Egg hatched at:Level 1, you need to have Met: (and whenever you submitted your team's app. For you, it would be 'Prior to M.6'
3. And last but not least, for Noah, you have question marks underneath the egg. You were supposed to leave that blank, since those are evolution spaces. When he evolves, you draw a headshot of the first stage, then if he evolves a second time, you draw what he evolved from. Example:
If I start as a Charmander. Then when I evolve into Charmeleon, I draw a headshot of what I looked like as a Charmander in the box below the egg. Once I evolve into Charizard, then below the Charmander box, I draw what I looked like as a Charmeleon. The only way you evolve is by completing missions and getting Evolution scrolls. Another thing. In the main box, every time I evolve, I have to draw my evolved form. So, in your case with Deino, when you evolve into Zweilous, you draw Zweilous in the main box, and Deino beneath the egg picture.
