I've had this lying around for ages and I figured I'd share it.

I have one note, I left the original base intact on the bottom layer so that you can use it as a guide for placing your OC's name. Due to the fact that I can't find a font that matches the one used on these I generally use that as a reference for the font and placement. I hope that helps.

My only rule is that you credit me when you use it, I'd love to see what you make!
