Since in which Lady Dimitrescu, Bela, Cassandra and Daniela had committed 27 crimes of murdering, punishing and torturing bad people, who were doing bad things happened. She and her daughters encountered The Descendants of God, Iron Guard, The Support Class and The Babysitters, who picked The Unnamed Nun and Eric from the New York Public Library after school, Brock and Lauren suggested to reject and pretend they do not want to see them. On Friday, Lady Dimitrescu decided to book a visit to The Liandri Grand Tournament secretly afterwards, she and her daughters robbed a bunch of sleeping splash potions, powdered dust, smoke grenades and Snipers' Jarates including the Elixirs of Holy All of The Same and Immortality (in which Lady Dimitrescu and her daughters had consumed all of them from the elixirs) and infiltrated the H.A.L.T. apartment tower, where they abducted the The Descendants, Iron Guard, The Support Class and The Babysitters before fleeing from the Team Fortress 2 team and brought the unconscious 18 of them in their castle.

Lady Dimitrescu decided to keep the anaesthetized Unnamed Nun in the Bedroom Chambers while her daughters imprisoned the rest of the 17 in the underground dungeon beneath the castle, which is somewhat is infiltrated by Ethan Winters, a young man sent by BSAA led by Chris Redfield in search of his baby daughter, Rosemary captured by Mother Miranda to use her as a vessel to resurrect her dead daughter, Eva, who is infected by the Spanish flu, which is forcing The Unnamed Nun to escape the castle, when she manages to find the missing masks to place the faces of the four angel busts on her own mission while Ethan rescues Eric, the Iron Guard, The Support Class and The Babysitters from the cellars and helped them find out where The Unnamed Nun went after correcting the puzzles, evading or killing Bela, Cassandra and Daniela, only to be tricked by Lady Dimitrescu realizing her real daughters are here that confused her for killing their "clones" after reaching the Tower of Worship to find a mysterious Dagger of Death's Flowers from a corpse in the casket before she is about to be raped by The Dimitrescu Family.

Ethan appears with the 17 saves her while they threw their flashbangs, smoke grenades and Jarates to distract Lady Dimitrescu and her daughters, who, Bela, Cassandra and Daniela combined with their mother and transformed into her mutated form of her in a dragon-like vampiric monster. After a battle while cursing Ethan and the 18, who survived the fall, she and her daughters crumbling to pieces, ending the Dimitrescu family once and for all. The group decided to help Ethan executed the rest of the Three Lords and Mother Miranda while he sacrificed his life to blow up the village with the Megamycete.

Back in the Unreal universe after The Descendants, Iron Guard, The Support Class and The Babysitters were recovered their injuries and unconscious effects however, The Dimitrescu Family revealed themselves to be alive after their apparent deaths since Ethan was presumed dead. Lady Dimitrescu suggested that she, Bela, Cassandra and Daniela decided to become good for their behavior to to watch over every universe to defend every good, punish the wrongs and sins for what have done to commit genocide, negativity, crime and blasphemy since after their pardon by themselves or Mother Miranda, who was already dead. They have no idea why Ethan is somehow still alive, suspicious and spying what The Dimitrescu Family is up to.

She inherited a specific disease in her blood from one of her ancestors, one which quite similar with Progenitor Virus symptoms-wise. She was taken into the village as she survived being infected with the Cadou parasite, and was given regenerative capabilities. She rules over a large castle as the Matriarch of the Dimitrescu Family, living with three women whom she considers her daughters, Cassandra, Bela, and Daniela.
Alcina mutated the three women, whom survived and became vulnerable to cold temperatures due to their mutated form now consists of flies spawned from Cadou eggs injected into their original human bodies. She, like much of the area, is a member of a dark cult who serve the being known only as "Mother Miranda", to whom she has loyalty to but also is not above acting on her own accord against her wishes. It was Mother Miranda who infected her with the Cadou Parasite, giving Alcina a host of extraordinary powers at the cost of needing to consume large amounts of human blood to sustain herself due to her blood disease.
Although Mother Miranda saw Alcina as a perfect candidate for a vessel for her late daughter Eva, but saw she wasn't perfect due to her insatiable quench for blood. Alcina would become a member of the Four Houses alongside Karl Heisenberg, Donna Benevieto and Salvatore Moreau, serving Miranda under their homes of the village. Despite the alliance between the four houses, Alcina frequently clashed with Heisenberg, seeing him as nothing but a child.
The Dimitrescu Castle became infamous, as many servants who came there wouldn't return, and the villages came to see the castle as a place of blood and death, and would avoid the area unless under extreme circumstances. Alcina would also use the blood of her victims to create a wine called Sanguis Virginis, and would often her victims that would become known as the Moroaicǎ and the Samca. Most of the Moroaicǎ were possibly former servants, considering that the Dimitrescu family were known to replacing maidens through the years.
Some of these maidens that were turned attempted to escape, most notably Ingrid, who was murdered and turned by Alcina when she attempted to leave the castle to the courtyard. In addition, Alcina feared that she would be harmed by the dagger of deathly flowers, and had it hidden in the family crypt while also having all the windows in the castle shut to protect her daughters."
