It's about people who kill themselves because they think it will help others, hence the living fish coming out of his mouth while he drowns. Sort of like this guy I met once who wanted to kill himself and everyone else because he believed the human race was wasteful and sick, or suicide bombers who think that their death will serve their country or their religion, or people who just feel so worthless that they think the world would be better off without them.
