Resembles a gigantic, bloated leech, its thick, glistening black hide rising bonelessly to a gaping maw lined with hundreds of serrated teeth that can shred a human body in seconds. It is a blind thing, but its other senses compensate, making it a deadly predator. . . . The creature possesses three snakelike grasping tentacles which extend from its underside just below the yawning orifice of its mouth.

These giant hirudineans are usually employed by Mythos forces to guard important artifacts and locations. The tentacles are venomous and skin contact can paralyse a human for 1-6 hours. A Watcher can move on land either by inchworming like a normal leech or by slithering on its belly like a snake, and it is most dangerous in the water. The creature's sight can only distinguish between light and dark but the watcher’s vibration and chemical senses are advanced. The massive leech is very hard to kill as it is immune to fire, have armor in its thick rubbery hides, can regenerate one hit point per round, and it flee when receiving enough damage.
