The reference was to the game Nuclear Throne~ Long story short, the max level you can get during a run allows you to get your Ultra ability, and it's symbolized mid-game by a crown icon appearing over the character's head for a moment. Similar to what was drawn here, though I think in-game it's colored in and not just an outline.
