I can just tell by the expression, which is both amused and creepy, that this guy is up to no good. I can imagine that he's thinking about what to do with his next victim. O_o Or is he possibly cutting himself? O_o O_o The back leaning pose is highlighting the amused aura around him.
I like the playful way how his holding the knife and the position also makes that sharp object stand out a bit more. The blood on his skin is nicely shaped and it's interesting that his elbows are stained/scratched like that.
