Gozu- originally he was gonna be like the Pyramid Head type character of the game idea, but I thought he'd be better as an enemy type than a boss. Gozu (aka Cow-Head) is based on two Japanese legends; one of two guardians of hell with the same name (the other being Mezu, aka Horse-Head) and the other legend being the one of the cursed story about a cow head or cow headed being, said to be so terrifying that it will induce night terrors, insanity and paranoia to anyone who reads it.
In the game, I thought Gozu could be the tormented soul of a demented serial killer butcher that skinned and ate women, and was possibly corrupted into doing such horrific acts by a manipulative person, either a fellow killer, cult leader or evil family member, that took advantage of his multiple personality disorder and schizophrenia in order to fulfil whatever sick reasons they had to have people killed. As a result of this mental torment, Gozu's soul was split into many, all killers and wielding halberds, possibly giving more to the disillusion that he is the guardian of the underworld.
They are definitely the more stronger enemies in the game that put up much more of a fight, with a variety of moves with axes, cleavers and halberds, and can also charge and gore with their cow horns.

Futakuchi-Onna- women with a second mouth on the back of their heads and tendril-like hair, these monsters are much like parasites that latch onto women that eat very little, as the mouth and hair have a mind of it's own, and take advantage of the starved woman to eat for itself. In the game, they are perhaps taken to a much more morbid extreme on latching onto women who have literally been starved to near death, either by means of a ceremony, or having been starved by a serial killer. As a result of the women becoming Futakuchi-Onna, they now have the ability to climb walls and ceilings and attack with flailing tentacles, that can tangle and ensnare enemies as well as whip them in rapid attacks. Many of the Futakuchi-Onna roam places where they starved to death and look like normal women wearing white kimonos, until they're approached or startled.
