I think this panel might have been a bit confusing in the actual comic (it's from page 12, here: www.deviantart.com/uhlrik/art/… ). Occasionally in the comic we'll see multiple Jokers in a panel, with the second being of either gender. They might be hallucinations on her part, manifestations of her fractured psyche, or imagine spots. Anyway, she's in the middle of getting dressed in the look she'll be wearing for the rest of issue 1 and into issue 2.
