She's looking up at the top of the staircase, and sees him. He's glaring at her, and she knows she's his next victim. He's coming...
Actually, I'm reminded of a scene from a Dracula comic book I read as a kid. A peasant girl is on her way home, walking through the forest, when she sees a large bat in the moonlight coming towards her. She sees it seem to change shape as it gets closer, then realizes that it IS changing, into a MAN! Too late, as Dracula, now in human form, lands on her, driving her to the ground where he proceeds to satisfy his thirst. This girl's upward gaze, and look of horror, could be that peasant girl.
