To me, the smoking strengthens the image of an extrovert.
He is not really concerned with inner struggles but with life around him (also enhanced by the outdoor scenery).
This image illustrates how some seek treasure inside, while others look for it externally.
Or it is just a pretty neatly coloured picture of a guy smoking. Yes. Who knows?
Art usually doesn't contain its own meaning - we attribute that to it ourselves.

As far as I can tell, there is no concept. The pseudo-artistic nature of the title is obvious when you change it to its polar opposite, "Introverted" - no artistic value is lost. Arguably, that title would be even more artistic, what with the smoke covering his face, hiding him from others.
Either that, or I just don't get it.
