The picture says it all.You may be wondering what happens with each option. It's simple really.
YouΒ Did (Choosing this means I made it from now on.) - You TF (And I pick the result)
I Did (Choosing this means you made it from now on.) - I TF (And you pick the result)
Unknown - We both TF (The result is randomized, so please don't ask for a specific result.)
I will restrict things a little:
No 18+
No MLP (I have nothing against it.)
Please ask about certain characters.

Update(s): Thought I should mention this. If you made the app, you control the TF!
I've changed the options a bit in the description due to my own confusions. People saying "I Did" sometimes has me wonder if they mean that I made it orΒ you did. So, from now on, those options are swapped. For those of you not seeing this (aka the ones I haven't responded to yet due to the confusion), I will be asking which specifically you mean.

*Your hair grows longer and pink, and a face starts to form on your belly*

*The face fully forms on your stomach, looking like a common jack-o-lantern face, and the transformation completes with your eyes becoming fully yellow*

"Well, not to brag, but I'm pretty sure I've got the only Pokeball so far. If anyone comes after ya, I could probably just use the app on them."

I tap its icon.

*I look over*
"Can't say I have."

I walk over to you and show you as the app opens.

"Huh...interesting."
*You start to grow white fur*

"Yea- huh?" I stare at a patch of growing fur.

*It spreads to your arms and legs, yet stops at your wrists and ankles, fluffing out*

I stare in confusion at my fur. "What is... the meaning of this?"

*Just shrugs*
*Your hands and feet cover in the same fur, and your nails extend out into long, black claws*

"You seem awfully composed about this!" I yell, glaring at my hands.

*the fur fluffs up on the backs of your legs, you fall on all fours, and the fur spreads up your head*

I clench my eyes shut, my heart racing. "Seriously?!"

*Your face turns a dark gray, and you start to grow two horns, one small, one large*
