You have to know the code.
:iconimhappyplz : (without the space) =

You put different stuff in between :icon and plz:
Like :iconmustacheplz : =

Once you've put in the word, click on "preview". It will then show your comment like it would be posted. If you did the icon code right, then it should show the icon.
There are a lot that don't exist, so if they don't show up then that's probably why.

I'm not sure if you mean how to show somebody's avatar on a comment, or how to get an avatar. So I'll just tell you both!

To show someones avatar on a comment, you put in, :iconperson'sname: Like this. :iconVypor : =

For actually getting an avatar for yourself. You have to have a picture that is no larger than 50x50 pixels.

It isn't hard to make one for yourself on Ms Paint if you have it. Just go to image then attributes and put 50 in both little boxes and click OK.

Then you just draw in the box, save, go to DA personal settings, and upload as your avatar.
