Yeah! It's weird, I know pffhahaha- Let me give you a summary of their differences.
[Top Right] Swap is the Underswap Sans who's friends with Dream! They hang out all the time, and Swap frequently travels between his timeline and the Doodlesphere where Dream lives.
[Center] Blue is the Underswap Sans who's been timeline hopping for years with his friend the human (soul)! He used to pretend to be a Classic Sans to evade someone but he no longer feels as in-danger as he once was. He still wears the jacket though.
