I got a 12 inch hollow half sphere of styrofoam and cut out the face. Then I glued a sheet of white foam inside for the mouth (and used sharpie for the teeth) and a thin piece of black nylon for the eyes and nose. I attached a strip of elastic along the back to keep it on (you could see the back of my head so I wore the hood over it most of the time)
I hope that made sense...I'm not very good at explaining things xD
