usually, its drawn then scanned and finished in Adobe Photoshop or done in Adobe Illustrator. Illustrator works better with these.

Have you seen "Paprika", the japanese movie ? One may think your pic has just been inspired or extracted from this movie...very similar, about dreams mixing in reality.
