He is normally always covered in bumps, cuts, scars, blood, wounds etc etc. He gets bullied a lot. Maybe something like dogs attacking or bullying him. Dark city scene with lots of busheling and bright signs etc. Or if not clearly anything else will be okay to xD he's a very flexible character.
