Well the geography is different, there is 7 nations, each with a theme (Asian, futuristic, military, modern, classic European...), and some of them, specifically the European one, have fantasy elements to them, such as elves, dwarves or vampires.
However, the biggest difference is ; the inhabitants of "Miraland" (the other world) solve every conflict with styling contests (there is a reason for that but it's a little complicated). Their entire society is based on styling. The better your styling skills, the higher your social status. The Nikki franchise are dress-up games.
