Actually it's an even WEIRDER story.
When CAPCOM was play testing Final Fight for North American release one of the play testers asked whether or not it was appropriate at the time to promote violence against women in the game (given at the time you only had male protagonists laying down the beats), the response from CAPCOM was that she was actually a transvestite (not any more appropriate but I guess it was ok as the arcade version released as is). When the game was ported to SNES both Poison and Roxy (? I think that was her twin's name?) were changed to a couple of dudes. Either way the lore stuck and has been one of those secret weird things in the CAPCOM universe.
