Japan just tends to be a bit more upfront when it comes to these things while the US and other PAL territories aren't. All it is is different viewpoints between Eastern and Western cultures.

Because Vivian was never male - she was transgendered.
I guess the dialog couldn't get that across too well without having fundie idiots picketing shit, so the implications were nixed.
