They were planning to build a blanket fort, but Deryn was all "WAIT! We must DESIGN it first!" and whipped out her sketchbook, and Alek of course said "I can't design things with my shirt on!" so he took that off, and as they were planning out all the nooks and crannies and whoseamawhatsits Alek was having difficulty seeing it from his angle, so he snuggled up behind Deryn for the best vantage point
Either that or she's saying "This is what I'm going to do to you" because to be quite honest that's what it looks like

I thnk that it started out as Deryn simply sketching Alek, who got carried away while modeling and distracted Deryn son much that she finally said that he could try drawing her without any clothes on and see how distracting it was and then...Alek took her up on the offer.

Yeah, the community screwed me up too. I can think of like, three or four dark, freaky things that prequeled this scene.
